Going abroad for business or pleasure? You may need travel vaccinations, travel advice and/or bite prevention advice, depending upon where you intend to visit. To help us offer the best protection you need please complete the questionnaire below - ideally 12 weeks prior to your travel.

Following completion of this form please bring it into the surgery and then contact us in 10-14 working days on 01977 611023 where you will be advised if you require an appointment for travel vaccinations or a telephone consultation for advice prior to your trip.

Some vaccinations are available free of charge from your GP, others have a charge and you may be required to attend a private travel health clinic so please complete your form ASAP.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
