GP Care Wakefield
GP Care Wakefield is an extended out of hours service owned by all Wakefield GP Practices to cover the entire poulation of the Wakefield District.
It Operates from 6-10pm Monday to Friday, and 9-3pm Saturdays, Sundays and Bank Holidays. Telephones will be automatically diverted during operating hours. Outside of these times please telephone 111 or ring back during normal surgery hours if your need is not urgent.
Following triage of your problem, if deemed appropriate the service will offer you a GP appointment at Trinity Medical Centre or Pontefract General Infirmary.
There needs to be shared consent in place for the service to be able to provide safe care therefore please ensute you give explicit record sharing consent when accepting an appointment with the service. Patients who prefer not the give shared consent cannot access this service.
The service is not available for routine matters such as repeat prescriptions.

The CCG (as part of the Connecting Care Partnership) is currently undertaking engagement focusing on self-care and has produced a short survey for Wakefield district resident’s to complete.
As you know self-care includes all the things people do independently to recognise, treat and manage their health.
This includes things like:
- Treating minor ailments such as coughs, colds and tummy upsets
- Managing a long term health condition
- Maintaining a healthy and balanced diet
- Getting enough exercise
- Stopping smoking
- Knowing where to find information or local support that can help
- Taking steps to improve your mental wellbeing.
We need to understand how self-care can improve people’s health and what support we should provide to help them to look after themselves. We also want to know how they feel about sharing their care records. They can let us know what they think by filling in our short questionnaire.
The Survey will close on 8th December.
